Sugar Plastic Surgery is committed to protecting the personal information of website users, and complies with the personal information protection provisions under the Act on Promotion of Information and Communications Network Utilization and the personal information protection guidelines established by the Ministry of Information and Communication. General Provisions
Article 1 (Consent to Collection of Personal Information)
When registering as a member, users are given the option to either agree or disagree with the Terms of Service. Agreeing to the Terms is considered consent to the collection of personal information. The hospital will take the necessary measures to allow withdrawal of consent to personal information collection through the same method and procedure used at the time of collection.
Article 2 (Purpose)

These Terms are intended to comply with and set forth matters related to the conditions and procedures for using the services provided by Sugar Plastic Surgery, in accordance with the Telecommunications Business Act and its Enforcement Decree.
The purpose includes identity verification and member management for the purpose of providing services upon membership registration.
The purpose is to fulfill the services provided by Sugar Plastic Surgery (medical consultations, appointment scheduling, informational materials, event information, etc.).
The hospital operates a channel through which you may raise opinions or complaints related to personal information protection. Any complaints regarding personal information should be directed to the hospital's personal information manager, who will take action immediately upon receipt.

Article 3 (Effect and Amendment of the Terms)

Personal information will not be provided to or shared with any third party or organization without the user's consent. If sharing with an affiliated partner is necessary to provide better service, individual notice will be given to obtain consent; if you do not consent, the information will not be shared with the affiliate.
Users may check their personal information at any time and may modify or delete it if they wish.
Your personal information will be destroyed once the purpose of collection or the purpose for which it was provided has been achieved, as described below. If information must continue to be retained, the user's consent will be obtained.


Membership registration information: when membership is withdrawn or the member is expelled
Medical fee payment information: upon full payment of medical fees or expiration of the statute of limitations for the claim
Medical treatment information: upon delivery or provision of the medical treatment or service

Article 4 (Service Withdrawal, Re-registration, and Restriction of Use)
Service Withdrawal
(1) If a user wishes to withdraw from the service, the member must personally request cancellation directly from the operator via email.
(2) When requesting withdrawal, the member must provide their name, resident registration number, ID, phone number, and reason for cancellation; membership will be cancelled after verifying that this information matches the member records.
(3) Withdrawal is confirmed once login with the existing ID and password is no longer possible.
Service Re-registration
A user who has withdrawn from the service and wishes to re-register may do so by personally requesting re-registration via email.
Restriction of Service Use
The company may terminate the usage agreement or suspend service use for a specified period without prior notice if a member engages in any of the following:

Planning or carrying out criminal activity through use of the service
Causing disadvantage to another person
Duplicate registration by the same user under a different ID
Actions that undermine sound use of the service
Any other violation of applicable laws or the terms of use set by the company

Article 5 (Measures to Prevent Leakage of Personal Information)
In using members' personal information, Sugar Plastic Surgery makes the following efforts to ensure security and prevent theft, loss, or damage of personal information.

Your personal information is protected by your ID and password, and can only be modified or deleted by the system administrator or by yourself; files and data transmissions are encrypted and protected through separate security functions.
Sugar Plastic Surgery uses antivirus programs to take measures to prevent damage to users from computer viruses.
Sugar Plastic Surgery uses encryption algorithms for IDs and passwords to safely handle personal information over the network, preventing leakage of your personal information due to hacking and safely defending against intrusion from outside.

本約款は、シュガー美容外科が提供するサービスの利用条件および手続きに関する事項、その他必要な事項を電気通信事業法および同法施行令の定めに従い遵守・規定することを目的とします。
会員登録時の本人確認および会員管理を目的とします。
シュガー美容外科が提供するサービス（医療相談、診療予約、案内資料、各種イベント情報等）を履行するための目的とします。
病院は個人情報保護に関してご意見・苦情を提起いただける窓口を設けており、個人情報に関するご不満がある方は病院の個人情報管理責任者までご意見をお寄せください。受付次第、直ちに対応いたします。

第3条（約款の効力と変更）

利用者の同意なく第三者や他機関に提供・共有されることはなく、より良いサービス提供のため提携先と共有する場合は個別に通知して同意を得る手続きを経ます。同意されない場合は提携先と共有いたしません。
利用者はいつでもご自身の個人情報を確認でき、ご希望の場合は修正または削除することができます。
お客様の個人情報は、以下のとおり収集目的または提供を受けた目的が達成された場合に破棄され、継続保有が必要な場合は利用者の同意を得ます。


会員登録情報：会員を退会または除名された時
診療費支払情報：診療費の完済日または債権消滅時効期間の満了時
診療情報：医療またはサービスが提供・引き渡された時

第4条（サービス退会、再登録および利用制限）
サービス退会
（1）利用者がサービスの退会を希望する場合、会員本人が直接メールにて運営者に解約を申請する必要があります。
（2）退会申請時には本人氏名、住民登録番号、ID、電話番号、解約理由をお知らせいただき、会員記録との照合確認のうえ退会処理を行います。
（3）既存のIDとパスワードでログインできなくなった時点で退会が完了したものとします。
サービス再登録
サービスを退会した利用者が再登録を希望する場合は、会員本人が直接メールにて再登録を申請してください。
サービス利用制限
会社は、会員が以下の事項に該当する行為を行った場合、事前通知なく利用契約を解除、または期間を定めてサービス利用を停止することができます。
1）犯罪行為としてサービス利用を計画または実行した場合
2）他人に不利益を与えた場合
3）同一利用者が異なるIDで二重登録を行った場合
4）健全な利用を妨げる場合
5）その他関連法令または会社が定めた利用条件に違反する場合
第5条（個人情報流出防止対策）
シュガー美容外科は、利用者の個人情報を取り扱うにあたり、盗難、紛失、毀損されないよう安全性確保のため、以下の方法により努めています。

お客様の個人情報はIDとパスワードによって保護されており、システム管理者およびご本人のみが修正・削除可能です。ファイルやデータ送信時には暗号化を行い、別途のセキュリティ機能を通じて保護しています。
シュガー美容外科はワクチンプログラムを利用し、コンピュータウイルスによる利用者の被害を防止するための措置を講じています。
シュガー美容外科はIDおよびパスワードに暗号アルゴリズムを利用し、ネットワーク上の個人情報を安全に取り扱っています。ハッキング等によるお客様の個人情報流出を防止し、外部からの侵入を安全に防御しています。
